HT Home Purchase Program Document Checklist

Preferred method to apply:
All of your documents can be uploaded and shared quickly and securely through
the online portal here: www.unhs.neighborworkscompass.org. Please make sure to update your profile information in your compass account.

Alternative method to apply:
Paper applications available on request.

Proof of Household Income:
Submission of all household income sources, including but not limited to:

 Most recent pay stubs for all household members aged 18 and above.
 Award letters for Social Security, Social Security Disability, or Workers Compensation.
 Documentation of child support payments or alimony, if applicable.

Proof of Residence in the City of Utica:
Verification of current rental status within the City of Utica through:

 Lease agreement.
 Signed statement from the landlord confirming residency.

Proof of Financing

 Preapproval from a financial institution for a minimum loan amount of $150,000.
 Preapproval must have been issued within the last 60 days.

Veteran or Disability Statement:
If applicable, applicants are requested to furnish a statement:

 For households with a veteran member, a written statement affirming veteran status.
 For households with a member with a documented disability, a written
statement confirming the presence of disability without the need for disclosing its nature.
